With access to secure chat, patients are able to coordinate directly with their care managers in a variety of ways including: scheduling, appointment reminders, quick check-ins, asking/answering questions, and collaborating across all care team members – nurses, community health workers, pharmacists, social workers, and behavioral health team members.
